Personal Data Request Notice - Promise

The City of South Salt Lake is committed to protecting your privacy and handling your data transparently. Below is a summary of the personal information we collect, how we use it, and your rights regarding your data. 

What is Personal Data?

The Utah Government Data Privacy Act defines “Personal Data” as “information that is linked or can be reasonably linked to an identified individual or an identifiable individual.” 

What Personal Data do we collect?

Afterschool and Summer Program Registration: Student Name, DOB, student number, gender, school grade, school registered, address, guardian name, guardian email, guardian phone number, guardian gender, alternative pickups name, alternative pickup relationship to child, alternative pickup phone number, race/ethnicity, home country, free/reduced lunch status, siblings and grades, languages spoken at home, refugee status, allergies, dietary needs, medication, IEP/504. 

  • Adult Program Registration: Name, email address, gender, pronouns, DOB, phone number, work phone, address, emergency contact. 
  • Housing Navigation Registration: Name, email address, race, gender, pronouns, DOB, phone number, work phone, address, emergency contact, financial information including, recent paystubs, year to date profit & loss statements (self-employed), most recent three years of tax Returns, W-2s or 1099s (If applicable), last three (3) months of bank statements, recent credit report, employer information, income, debt. 
  • Childcare Permission: Name, email address, gender, pronouns, DOB, phone number, work phone, address, emergency contact, child name, child age.
  • Promise Survey: Guardian Name and Email

Why do we collect this data?

We collect personal data in order to provide and improve our services; communicate about updates, events, and opportunities; to ensure safety and security for PSSL program participants and comply with child care licensing; and to comply with funding and legal obligations.

For over a decade, Promise SSL has worked alongside schools and community partners, to improve outcomes for students and families on a community-wide scale. Through aligned goals, strategies, and data sharing, we are working towards ensuring every child and family has the opportunity to succeed. 

Community-level and individual-level information is central in informing the work of Promise SSL. It helps us understand school and community issues, identify solutions, commit to meaningful action, coordinate efforts, and assess the effectiveness of our programs and strategies. 

Having the ability to track and report on individual and program successes is crucial to receiving funding and resources in order to provide high-quality programs which improve outcomes. Student’s education and related information – including demographics, attendance, grades, and test scores – is used to evaluate student success and needs, tailor services, assess program impact, and connect you and your child to additional resources that best serve you.

Who do we share your data with?

Student youth registration data is shared with Granite School District, Kearns St Ann School, Utah International, Utah Afterschool Network and Utah Education Policy Center for program outcome reporting purposes. 

De-identified aggregate data is shared with state, federal, and private funders, no personally identifiable information is shared.

Third parties that we partner with to provide such resource navigation and resources as you request, such as housing applications, food bank referrals, etc.

Funding stipulations require Promise South Salt Lake to participate in ongoing evaluations, which includes sharing student data including your child’s name, student number, and program participation with evaluators including Utah State Board of Education, 21st Century, and Department of Workforce Services. Evaluators use these data only for the purposes of fulfilling grant duties and do not share these data with any other third parties without your written consent. 

How do we protect your data?

Data is only shared with a select group of individuals if:

  • a Promise Partnership Data Consent form is signed
  • the Promise Partner has received specific training about how to keep information secure
  • the Promise Partner works in the student’s community, and
  • the Promise Partner needs to know the information to support the student’s success

We implement adequate security measures including encryption, to ensure the confidentiality and integrity of your data. You can request to see the data we hold about you. You can request that we correct any inaccuracies in your data. You can ask us to delete your data under certain conditions. Individuals may choose not to disclose information.
